Privacy choices
Regardless of where you live, you can ask us to confirm what personal information we hold about you and give you access to it, correct anything inaccurate, delete your information, or send you a portable copy of what you gave us. You do not need an account with us to ask.
What happens after you submit
- We verify your identity before we act, so we do not hand your information to someone else.
- We respond within 45 days. If we need longer we may take one 45-day extension, and we will tell you why.
- If we say no, you can appeal by replying to our response, and we will answer the appeal within 45 days.
Some information we have to keep — credentialing and assignment records, and taxpayer identification numbers for seven years after your last payment. The Privacy Policy sets out exactly what and why. Background check reports are held by Checkr, not by us; you can request yours directly from them.
You can also email [email protected]. Either way counts, and the date we receive it is the date the 45 days starts.
We honor Global Privacy Control browser signals as an opt-out request. We do not sell personal information or run targeted advertising.
If we deny an appeal, Colorado residents may contact the Colorado Attorney General at coag.gov.
